Case management is the coordination of services between a client and a practicing health care official. The process provides people with the guidance and support they need during trying times, and an effective case manager understands how his or her influence can help his or her clients succeed. Some case management skills are learned through life experiences, but there are basic values that any successful professional embodies from the start. These skills and values help promote client wellness.

Beyond being accountable for the entire case and acting as the primary contact, a professional undertaking these responsibilities assists each party in achieving agreed goals, determines provisional and ongoing liability and coordinates services to reach desirable outcomes.

Case managers are frequent interviewers, as understanding and managing a case requires professionals to obtain and provide a vast amount of information to all involved parties. It's important for case managers to be prepared for both informal and formal discussions.


As case management is a working relationship between a client and a professional, the ability to effectively communicate is paramount to a successful partnership. Managers need to convey and obtain accurate and complete information in a respectful manner, so understanding the pivotal role language plays in mentoring is important.


The case manager often acts like a mentor and helps clients build skills necessary to independence. If the manager is unable to provide this type of assistance, he or she connects clients with the proper resources that can offer further guidance.


The case manager needs to make sure the proper information is provided and obtained from a variety of parties. This professional must foster cooperative relationships with all resources within the environment, as he or she is expected to develop care plans that address all problems.


Case management is a delicate process. There are specific policies and procedures that must be followed to discharge or terminate the relationship between client and manager, and failing to follow these rules can lead to client abandonment. If a case manager fails to follow the guidelines and doesn't give the client proper notice about termination, malpractice litigation is a distinct possibility.
